NHS Criteria

• Under 16 years of age or Over 60 years of age
• Full time students aged 16, 17 or 18 attending School, College or University
• Patients with Glaucoma or Diabetes
• Over 40 years of age, with an immediate relative suffering from Glaucoma
• Receiving Income Support, Income Based Jobseeker's Allowance, Working Families Tax Credit NHS Exemption Certificate or Disabled Person's Tax Credit
• NHS Exemption Certificate
• Holder of a current HC2 Certificate
• Registered as sight impaired (partially sighted) or severely sight impaired (blind)
• Requiring Complex Lenses (determined by NHS guidelines)
